Third nerve palsies, also known as oculomotor nerve palsies or cranial nerve III palsies, refer to a neurological disorder characterized by a dysfunction of the third cranial nerve. The third cranial nerve, or oculomotor nerve, is responsible for controlling the movement of multiple muscles that control eye movement, including the superior rectus, inferior rectus, medial rectus, and inferior oblique muscles, as well as the muscle responsible for pupil constriction, called the sphincter pupillae.
When a third nerve palsy occurs, the affected individual experiences a range of symptoms, including a droopy eyelid (ptosis), double vision (diplopia), and an inability to move the affected eye properly in all directions. Other accompanying symptoms may include decreased pupil size (miosis), a dilated pupil (mydriasis), and an outward deviation of the eye (exotropia).
Third nerve palsies can be caused by various underlying conditions, including but not limited to head trauma, vascular disorders, aneurysms, tumors, infections, or diabetes. Treatment typically depends on the underlying cause and may involve addressing the root condition through medications, surgery, or vision therapy. In many cases, third nerve palsies improve spontaneously over time, especially if the cause is temporary or reversible. However, in certain cases, the condition may persist, leading to long-term functional and aesthetic impairments.
